CASE MANAGEMENT Solutions


Horizon Series was created as a software solution to the complex problem of data cooperation among justice and mental health partners. The Horizon Series co-responder case management software aims to grow through community-engaged research partnerships. Horizon Series reduces the costs associated with storing and managing process and outcome data and removes barriers to collaboration for crisis response programs.

Participating sites will benefit from access to a mobile-friendly, web-based software solution for crisis response programs. Our desire is to advance knowledge and to help programs reach their goals. Horizon Series has been funded by the Bureau of Justice Assistance through the Justice and Mental Health Collaboration and Second Chance Act awards and a Microsoft Catalyst and Urban Institute Justice Reform award. This project is sponsored by Virginia Innovation Partnership Corporation.

DESIGN PRINCIPLES


Co-responder tracks key process and outcome indicators at the individual-level that are specific to co-response settings. Horizon Series software has been adapted for justice diversion programs that range from co-response, crisis intervention training, residential reentry, and mental health wellness visits.
